Do patients with pilonidal disease prioritize better cosmetic outcome over recurrence risk?

Giriş

Pilonidal hastalık, akut abse, persistan akıntı, kıllar, sinüs açıklıkları, subkutan sinüs traktları ve skar dokusu ile karakterize kronik bir hastalıktır. Genel popülasyonda görülme sıklığı %0-%5 olup, hastaların çoğu 15-30 yaş arasındadır. Eksizyon ve primer kapama pilonidal hastalığın tedavisinde kullanılan, intergluteal sulkusta lineer bir skar bırakan, yüksek nüks oranı olan orta hat kapatma tekniğidir. Bununla birlikte, modifiye Limberg flep tekniği, gluteal bölgede z şeklinde bir cerrahi skar bırakır ve nispeten düşük (%0-%7.4) nüks oranına sahiptir. Bu nedenle pilonidal hastalığı olan bireylerin, ameliyat öncesi her iki girişimin kozmetik sonuçları ve nüks oranları açıklandıktan sonra, hangi tekniği tercih edeceklerini sorguladık.

Materyal ve Metod

Ocak 2018-Temmuz 2019 tarihleri ​​arasında, primer pilonidal hastalığı olan 19 hasta (15 erkek, 4 kadın), eksizyon+primer kapama sonrası nüks olan 4 hasta (3 erkek, 1 kadın) ve eksizyon+modifiye Limberg flep tekniği sonrası nüks eden 2 hasta (1 erkek, 1 kadın) cerrahi müdahale öncesinde kozmetik sonuçlar ve nüks oranları açısından eksizyon+primer kapama ve modifiye Limberg flep teknikleri hakkında ayrıntılı olarak bilgilendirildi. Tercih edilen cerrahi teknik ve hastanın bu tekniği seçmesinin nedeni kaydedildi.

Bulgular

10 hasta (7 erkek+3 kadın) daha az ameliyat izi bırakması nedeniyle eksizyon ve primer kapamayı tercih ederken, 15 hasta (12 erkek+3 kadın) nüks riski daha az olan modifiye Limberg flep tekniğini tercih etti. Evlenmemiş olan üç kadın hasta evlilik öncesi daha az ameliyat izi istedikleri için kesinlikle eksizyon ve primer kapama tekniğini tercih edeceklerini belirtti. Daha önce herhangi bir komplikasyon olmaksızın primer eksizyon ve kapama ameliyatı geçirmiş bir akrabası veya arkadaşı olan dört erkek hasta aynı cerrahi işlemi yaptırmak istedi. Pilonidal cerrahisi başarısız olan tüm hastalar (yara enfeksiyonu sonrası iyileşmeyen yara veya nüks), kozmetik sonucu tartışmadan bile daha düşük nüks oranı nedeniyle modifiye Limberg flep ameliyatını tercih etti.

Sonuç

Primer pilonidal hastalığı olan erkek hastaların çoğu ve rekürren pilonidal hastalığı olan erkek hastaların hepsi nüks ihtimali daha az olan modifiye Limberg flep yöntemini tercih etti. Eksizyon ve primer kapama tekniği daha yüksek nüks oranına sahip olmasına rağmen, evli olmayan kadın hastalar primer pilonidal hastalığın tedavisinde nüks riskindense daha iyi kozmetik sonuca öncelik verdiler. Ancak, pilonidal cerrahisi sonrası nüks görülen tüm kadın hastalar, nüks oranının daha düşük olması nedeniyle, modifiye Limberg flep tekniğini tercih ettiler. Bu verilerin ışığında primer pilonidal hastalığı olan kadın hastalar nüks riskinden ziyade daha iyi kozmetik sonuçlara önem verirken, rekürren pilonidal hastalığı olan tüm bireyler daha düşük nüks ihtimali olan operasyon yöntemini tercih etmiştir. Ancak daha büyük bir örneklem kümesi ile çalışılması istatistiksel olarak anlamlı sonuçlara ulaşılmasını sağlayacaktır.

Introduction

Pilonidal disease is characterized by acute abscess, persistent discharge, hair, pilonidal pits, subcutaneous sinus tracts and scar tissue. Its incidence in the general population is 0-5%, and most of the patients are between the ages of 15-30. Excision and primary closure is a midline closure technique, which leaves a linear surgical scar in the intergluteal sulcus with a high recurrence rate, for the treatment of pilonidal disease. Nevertheless, modified Limberg flap technique leaves a z-shaped surgical scar on the gluteal region with a relatively low (0%-7.4%) recurrence rate. Therefore, we questioned the patients with pilonidal disease prior to surgery, which technique they would prefer after the explanation of cosmetic outcomes and recurrence rates of both interventions.

Materials and Methods

Between January 2018-July 2019, 19 patients (15 male, 4 female), who had primary pilonidal disease and 4 patients (3 male, 1 female) with failed pilonidal surgery after excision+primary closure, and 2 patients (1 male, 1 female) with recurrence after excision+modified Limberg flap surgery were informed about excision+primary closure and modified Limberg flap techniques in detail about the cosmetic outcomes and recurrence rates prior to surgical intervention. The preferred surgical technique and the reason why the patient chose that technique were noted.

Results

10 patients (7 male+3 female) who were in favor of less surgical scar preferred excision and primary closure while 15 patients (12 male+3 female) patients who preferred less recurrence were in favor of modified Limberg flap technique. Three female patients who were unmarried stated that they would definitely prefer excision and primary closure technique as they wanted less surgical scar before marriage. Four male patients, who had a relative or a friend who had had previous primary excision and closure surgery without any complications, wanted to undergo the same surgical procedure. All of the patients with failed pilonidal surgery (non-healing wound after wound infection or recurrence) preferred modified Limberg flap surgery due to its lower recurrence rate, even without discussing the cosmetic outcome. 

Conclusion

Most of the male patients with primary pilonidal disease and all of the male patients with recurrent pilonidal disease preferred the modified Limberg flap method, which was less likely to recur. Despite the fact that excision and primary closure technique had higher recurrence rate, unmarried female patients prioritized better cosmetic outcome over recurrence risk for the treatment of primary pilonidal disease. Nevertheless, all of the female patients with failed pilonidal surgery preferred the modified Limberg flap technique due to its lower recurrence rate.

In the light of these information, female patients with primary pilonidal disease emphasized better cosmetic results rather than the risk of recurrence, while all individuals with recurrent pilonidal disease preferred the surgical technique with a lower probability of recurrence. However, further studies with a larger patient population will provide statistically significant results.

Does the Bathing of Covid-19 Infected Patients After Death Require Precautions to Limit the Spread of viral Infection?

Introduction: Coronavirus disease 2019 (COVID-19) is a viral infectious disease which has spread all over the world causing a pandemic. COVID-19 disease is extremely contagious, spreading mainly through respiratory tract secretions and air droplets. The progression of the disease which causes acute respiratory distress syndrome, mostly in elderly patients over 65, requires intensive care unit admission. Unfortunately, 97% of COVID-19 patients who were mechanically ventilated passed away (1).

In Muslim society, the corpse of the deceased is washed after death, before the funeral. The body is laid onto a large table in supine position, and the Imam washes the corpse throughly from head to toe with clean water, before placing  the deceased person into a white cotton cloth surrounding the whole body. However, if the deceased person is infected with Corona virus, the waste water from the bathing of the corpse mixes into the sewage system.

Discussion: City sewage systems in Turkey either end up in waste water treatment facilities or open sea. Thus, Corona virus may be carried along the pipeline through the city and pour into fresh water (2). Furthermore, Chamber of Environmental Engineers (UCTEA) in Istanbul published a report on the effect of COVID-19 on the management of waste water, in April 2020 (3,4). According to this report, fecal oral transmission of the Corona virus is possible, which explains diarrhea in the early phase of the COVID-19 infection (5). The viability of COVID-19 virus varies from several hours to several days (6). In addition, solid waste in the sewage system both produces a protective shell and forms a nidus for the virus. Moreover, some of the waste water after treatment may be used for watering of the public gardens, which may lead to the spread of the virus to both human beings and environment.

Conclusion: In the light of these information, we recommend Muslim community in the world to take precautions for the disposal of infected waste water used to bathe COVID-19 infected patients after death, in order to prevent the possible spread of COVID-19 infection.

Conclusions

Since we have demonstrated significantly low serum levels of ferritin and 25(OH)D in the patient group, we suggest that serum iron and vitamin D status should be evaluated in all patients with diffuse hair loss prior to further treatment. Vitamin B12 and zinc deficiencies were determined in a few patients. Therefore, it does not need to be mandatory to check vitamin B12 and zinc levels routinely in every patient complaining of diffuse hair loss.

Colonoscopy in patients with psoriasis before the initiation of treatment with biological agents

Abstract

Psoriasis may lead to subsequent colorectal cancer since chronic systemic inflammation is the common etiologic factor both in psoriasis and colorectal cancer. It is a matter of dispute whether biological agents used in the treatment of psoriasis cause predisposition to colorectal cancer as a result of their immunosuppressive effect. Medical records of psoriasis patients who underwent colonoscopy before biological agents were reviewed. Colonoscopy was performed in all patients who were aged 50 years and older and in patients younger than 50, if they had positive fecal occult blood test results. The study included 95 patients between the age of 34‐84. Colonoscopy results revealed tubular adenoma in 16 (16.8%) patients, hyperplastic polyps in 7 (7.4%) patients, villous adenoma in 1 (1.1%) patient and tubulovillous adenoma in 1 (1.1%) patient. Two patients were diagnosed with colon cancer detected by a former colonoscopy which was recommended by their dermatologist before the biological agent treatment plan. These results revealed that adenomatous polyps which can evolve to colon adenocarcinoma were the most frequent polyp type in patients with psoriasis. We suggest that routine colonoscopy should be performed before the initiation of biological therapy in psoriasis patients who are aged 50 years old and over.

Pilonidal disease, hidradenitis suppurativa, acne conglobata, and dissecting cellulitis of the scalp are the members of follicular occlusion tetrad with similar pathophysiology, which is initiated by follicular occlusion in apocrine gland bearing areas.Interestingly, while the treatment of hidradenitis suppurativa, acne conglobata, and dissecting cellulitis has been undertaken by dermatologists, the treatment of pilonidal disease has been left to general surgeons. However, pilonidal disease can also be treated by dermatologists with crystallised phenol using a minimally invasive technique in outpatient clinics.Hereby, we would like to explain crystallised phenol technique in detail in order to encourage dermatologists to treat pilonidal disease. The detailed visual description of crystallised phenol technique in the treatment of pilonidal disease is depicted in Figure 1.

The treatment of pilonidal disease with crystallised phenol has many advantages. This technique is minimally invasive; moreover, learning curve is extremely short. Only one-time application is enough to learn the whole procedure. There is no need for operating room settings. Dermatologists can apply crystallised phenol to their patients with pilonidal sinus in their outpatient clinics. Furthermore, patients can get to work after the procedure immediately. Possible postoperative complications, such mild pain, purulent discharge, and abscess formation, can be treated with non-steroidal anti-inflammatory drugs and antibiotics.Moreover, referring the patient to a general surgeon is always an available option.

The post-treatment cosmetic results are excellent, as there are no incisions or stitches after the procedure. Phenol treatment has a 30-40% failure rate with multiple and suppurative pilonidal disease. However, patients who have limited disease (1-3 pilonidal pits) have higher success rates.Our two-year follow-up of 50 patients with pilonidal disease treated with one-time application of crystallised phenol revealed 80% complete healing rate. Therefore, we encourage our dermatology specialist colleagues to treat patients diagnosed with pilonidal disease with crystallised phenol in their outpatient clinics.


Does Vitamin D Deficiency Cause Direct Inguinal Hernia? Preliminary Results

The main reason for the inguinal hernia development is the degenerative changes of the lower abdominal muscles that weaken the muscle strength. Myocyte atrophy, fibrosis and fatty degeneration in the internal inguinal ring area and the abdominal wall have been demonstrated previously (9). As the weakness in the connective tissue or muscles of the patient leads to an inguinal hernia, we suggest that decreased vitamin D levels may be associated with the development of a direct inguinal hernia. In our study, the patients with direct inguinal hernia had lower serum 25(OH)D levels (23.11±8.42) than in healthy individuals (24.77±10.67). There was no statistically significant difference in vitamin D levels in both groups (p=0.77). However, low vitamin D levels were more frequent in patients with inguinal hernia compared to healthy individuals. Twenty-seven (90%) patients with inguinal hernia had serum vitamin D levels under 30 ng/mL, while 21 (70%) healthy individuals had serumvitamin D levels under 30 ng/mL. The risk of vitamin D deficiency increases by age as in inguinal hernia development (7, 9). Our study demonstrated that low serum 25(OH)D levels are common in elderly patients with direct inguinal hernia.

Conclusion: Decreased levels of vitamin D may facilitate the development of an inguinal hernia. Our results revealed that low serum vitamin D levels are more common in patients with direct inguinal hernia compared to healthy individuals. However, there was no statistically significant difference between the two groups. Therefore, the vitamin D status of a larger sample group should be evaluated in order to reach a definitive conclusion.








Pilonidal disease specimens of 905 patients revealed no malignancy, however we still insist on histopathological examination

Abstract
Aim: Pilonidal disease presents with acute abscess, sinuses, pits and scarring in the sacrococcygeal region. Surgical intervention is one of the treatment options of pilonidal disease. However, it has been a matter of debate, whether histological analysis of surgically resected pilonidal disease specimens would be necessary.
Material and Methods: Medical records of the patients with pilonidal disease who underwent either primary excision and closure or modified Limberg flap surgery were reviewed retrospectively. The results of the histopathological examination of the pilonidal disease specimens were evaluated.
Results: This study included 905 patients (686 male and 219 female) with pilonidal disease who were treated with surgical intervention. The mean age of the patients was 23.96±7.5 years. All surgically excised pilonidal disease specimens of the patients were histopathologically evaluated in order to rule out malignancy. No malignancy was detected.
Conclusion: We conducted a Pubmed search with the term “pilonidal” between January 2000 and March 2019. We identified 91 patients with malignant degeneration arising on pilonidal disease in the English medical literature. The most common pilonidal disease associated malignancy was squamous cell carcinoma. Even it is rare, malignancy may accompany pilonidal disease. Therefore, even we have not encountered any malignancy in 905 patients with pilonidal disease, we still insist on histopathological examination of all surgically resected pilonidal disease specimens in order to rule out malignancy.

Abstract
Aim: Primary pilonidal disease can be treated with crystallized phenol with an overall cure rate of 80%. However, the effect of crystallized phenol in patients who had recurrence after pilonidal disease surgery has not been analyzed in the English medical literature in detail yet. The purpose of this study was to investigate the effect of crystallized phenol in recurrent pilonidal disease.
Material and Methods: 28 male and 10 female patients who had recurrence after surgical intervention to treat pilonidal disease were evaluated. Twenty male and 8 female patients underwent excision and primary closure surgery, whereas 8 male patients and 2 female patients underwent modified Limberg flap surgery. All of these 38 patients were treated with one-time application of crystallized phenol.
Results: The mean period for recurrence of patients treated with single application of crystallized phenol after primary excision and primary closure surgery was 45.85±11.82 days (range: 35-90 days) and it was 66.5±25.31 days (range: 40-120 days) after modified Limberg flap surgery. The follow-up period was 6 months. The cure rate in recurrent excision and primary closure group was 57.14%, whereas the cure rate in the recurrent modified Limberg flap group was 40%. Overall cure rate was 52.63%.
Conclusion: As a result, crystallized phenol treatment was less effective in patients who had recurrence after pilonidal disease surgery in comparison to patients who were only treated with crystallized phenol without prior surgical intervention.

Pre-treatment vitamin B12, folate, ferritin, and vitamin D serum levels in patients with warts: a retrospective study

Aim To compare the serum levels of 25-hydroxyvitamin D, ferritin, folate, vitamin B12, zinc, and thyroid stimulating hormone between patients with warts and healthy individuals. 

Methods This retrospective study enrolled 40 patients with warts and 40 healthy individuals treated at the Ufuk University Hospital, Ankara, between July and December 2017. Serum levels of 25-hydroxyvitamin D, ferritin, folate, vitamin B12, zinc, and thyroid stimulating hormone status were evaluated retrospectively. Results Participants with and without warts had similar mean serum 25-hydroxyvitamin D, ferritin, folate, zinc, and thyroid stimulating hormone levels. However, patients with warts had significantly lower mean serum vitamin B12 level (P=0.010). Patients with warts non-significantly more frequently had decreased serum levels of 25-hydroxyvitamin D, ferritin, and folate (P=0.330, P=0.200, P=0.070, respectively). 

Conclusion Patients with warts may require evaluation of serum levels of vitamin B12, folate, ferritin, and vitamin D.
